Under the general direction of the Director of Nursing, the Registered Nurse is primarily responsible for facilitating positive Individuals' health care outcomes for assigned locations via provision of clinical oversight to LPNs, MDSP, on-call responsibilities, staff training, advocacy, coordination of health care services and communication with clinical and administrative staff.
Essential Functions:
To remain on-call via cell phone Responds within 10-20 minutes.
Responds to all calls.
Demonstrates good judgment.
Completes Triage E-Mail Provides clinical oversight for assigned locations.
Reviews, signs and makes recommendations at least monthly for all documents pertinent to consumer health.
This includes, but is not limited to, nursing notes, doctors' orders, appointment forms, labs, vital signs, weights, annual/semi-annual reviews, hospital reports, day program documents and incident reports.
Meets with LPN/MDSP to review findings and recommendation at least 2x per month.
Contacts LPN/MDSP immediately if findings and/or recommendations are critical to individual's health or safety and provide instructions.
Monitors known conditions.
Manages notifications to proper authorities detailing updates in individuals' medical conditions.
Ensures compliance with OPWDD Regulations and agency policies and procedures.
Manages medications to ensure adequate supply, current prescriptions, and that medications are administered correctly.
Manages MARs monthly.
Manages PPDs for new employees.
Acts as a liaison with medical providers.
Tracks, develops, and initiates staff training.
Monitors staff AMAP dates and schedules recertification courses prior to expiration.
Promptly arranges supervised pourings following AMAP classes based on the needs of the residences (gives priority to shifts most in need of certified staff).
Initiates more frequent supervised pourings with staff who have a history of medication errors.
Trains on infection control and OSHA at least annually and as needed Develops training based on feedback from team members.
Assists with in-house orientation training for new staff.
Develops and initiates training specific to individuals' conditions/needs (trains on Nursing Care Plans).
Demonstrates proper protocols for staff.
Maintains Nursing Plans of Service, updates as needed, and are available to staff for review.
Utilizes OPWDD curriculum to train staff on diabetes and tube-feeding at least annually where applicable, and as needed for new staff.
Documents trainings including a brief description of what was covered, obtains staff signatures and dates.
Advocates for optimal consumer health care.
Attends Inter-disciplinary Team Meetings.
Presents on priority medical issues at team meetings.
Recognizes signs and symptoms of possible conditions and advocates accordingly.
Revokes AMAP certification from staff deemed unsafe to pour medications/documents justification.
Monitors staff implementation of health care protocols.
Visits hospitalized consumers to ensure needs are being met and to advocate accordingly.
Advocates for and attends hospital discharge planning meetings.
Conducts timely assessment of individuals following injuries and at other appropriate times.
Follows up on telephone triage by visually inspecting individual within a reasonable amount of time (generally 24 hours).
Documents consumer assessment appropriately, providing specific details (i.
e.
diameter of bruise, ROM, response to probing, etc.
) and all information relevant to the outcome of the assessment.
Evaluates and assesses consumers within 24 hours of hospital discharge.
Monitors resolving conditions.
Completes and documents body checks at least monthly for individuals deemed at high risk for skin breakdown.
Serves as a positive role model for staff.
Demonstrates a positive attitude.
Represents EFLI in a courteous and professional manner.
Interacts with consumers in a friendly, respectful manner.
Develops rapport with staff.
Works with Management and Administrative staff to present a consistent and united front to staff in support of the agency mission and a positive corporate culture.
Communicates effectively with supervisor.
Provides updates on individuals' medical status.
Reports any concerns based on clinical review or observation that could compromise consumer care to supervisor and the team.
Reads and responds to e-mail daily.
Develops individualized Nursing Plans of Service for each consumer.
Updates plan at least yearly and as needed if there is a change of status.
Provides specific instructions for staff through Nursing Care Plan Addendums.
All other duties as assigned by supervisor.
Qualifications:
NYS RN license is required A valid/clean NYS driver's license is strongly preferred Flexibility to provide support to program sites as needed OPWDD program experience or nursing home a plus This position offers:
